Person Centred Planning

Helping folks thrive.

MyCompass makes it easier to co-create detailed care plans, big picture life goals and all the things people served, families, staff and even government funders need to know to help folks to thrive.

THe challenge we solve

Helping voices be heard

For decades now, social service agencies have talked about being alongside people they serve to center their needs, wishes and goals in care planning. Aspirations have been high, but consistently people served have told service providers and governments that often their voice and ideas get drowned out by the one-sided needs of service systems. As a result of past planning practices, people served often feel left out, uninspired by goals and left with a feeling that planning is for the service provider and government and not really for the people the system is meant to serve.

MyCompass asked, How can we make it easier to center the needs of people served and help service providers and government funders get their needs met without getting in the way?

Plan together

You can plan in real-time with MyCompass in a living room, coffeeshop, office or anywhere with internet connection. But sometimes people don't have time to all meet at the same time. To help with co-creation even when planning at different times, MyCompass created a digital space where a trusted facilitator can set up the plans with goals based on conversations they've started and then people served and families can log in anytime that works for them to add their ideas, comments and goals they want to make sure are not left out.

Accountability made easier

MyCompass goal planning helps you create the goal, create tasks to accomplish the goal, set a due date, assign and notify staff, and family members who will help accomplish it and track real time progress. MyCompass wanted to make sure ideas and goals don't get forgotten and help make it clear who will help a person served to accomplish their goals. Notifications get sent and automatic follow ups get sent before a goal is due to help ensure progress is happening. This tracking of goals, learning and accomplishment also turns into reports to help with funding accountability where needed.

Goal inspriation when you need it

In MyCompass we have goal libraries within each of the 6 core pillars. The goal libraries help people and staff to see ideas that might inspire them. Each goal when selected also populates specific tasks to help accomplish the goal. The goal library can be customized by agency leads when you purchase MyCompass.

5 Core Planning Pillars

In our 5 years of research, people served, service providers and government told us the following are the core pillars of person-centred planning where goals get explored.
These pillars are:
Home Life
Strengthening Relationships
Health and Wellbeing
Community Connections

Custom pillar options

Some organizations want to create extra pillars and craft their own questions that people served and facilitators of planning can answer. When you purchase MyCompass your agency leads can learn how to change these pillars and the questions yourself. For example one organization wanted a pillar called Learning and crafted questions around ways to supporting continuous growth and learning

Stretch goals or maybe just a goal that maintains the good stuff

In MyCompass when a goal is set, there is question asked around whether the goal is a stretch goal, a quick win or a goal that maintains quality of life. These questions help nudge everyone involved to both plan for the present needs and think bigger because often when we stretch a little beyond our comfort zones good things happen. Little nudges in planning process like this, helps to make it easier to ensure care planning meaningfully supports people to thrive and not just survive.
